Upper back muscle tension often develops from a combination of daily habits, movement patterns, stress, and compensation from nearby areas. Common causes may include prolonged sitting, forward-head posture, rounded shoulders, computer or phone use, stress-related muscle guarding, neck pain compensation, shoulder weakness or irritation, repetitive lifting or caregiving, driving posture, poor sleep position, breathing pattern changes, upper back trigger points, and prior injury or whiplash.
Muscle tension can become chronic when the nervous system stays in a protective pattern. In these cases, the muscles may feel tight not because they are 'short,' but because the body is guarding, stressed, irritated, or compensating.
The upper back is a busy crossroads connecting the neck, shoulders, ribs, and thoracic spine. When one area is not moving well, another area often compensates. Limited thoracic mobility may increase strain on the neck and shoulders. Shoulder irritation may cause upper back muscles to tighten protectively. Stress may increase muscle tone throughout the neck and upper back.
