Thoracic pain refers to pain in the upper or mid-back region, generally around the rib cage, shoulder blades, and thoracic spine. Some people describe it as pain between the shoulder blades. Others feel stiffness, tightness, burning, aching, or sharp pain with twisting, breathing deeply, sitting, lifting, or reaching. Thoracic pain can develop from overuse, stiffness, poor posture, injury, muscle tension, or compensation from nearby areas. Common causes may include muscle strain, poor posture or prolonged sitting, rib joint irritation, shoulder mechanics or compensation, neck pain affecting upper back muscles, repetitive lifting or caregiving, golf or pickleball rotational strain, arthritis or degenerative changes, stress-related muscle guarding, trigger points in the upper back, and reduced mobility in the thoracic spine.
Thoracic pain should be evaluated carefully because pain in the upper back or chest wall can sometimes overlap with heart, lung, digestive, or other medical concerns.
The thoracic spine connects to the rib cage and supports upper-body rotation, posture, and breathing mechanics. When this area becomes stiff, the neck, shoulders, and low back may compensate. Limited thoracic mobility may contribute to neck strain from forward-head posture, shoulder tightness with reaching, pain between the shoulder blades, reduced rotation during golf or daily movement, rib tightness with deep breathing, and increased stress on the low back.
Thoracic pain often improves best when care looks at the full movement system — including upper back muscles, rib mobility, shoulders, neck, breathing patterns, posture, and daily activity demands.
Medical acupuncture may be used as part of a conservative care plan for thoracic pain. Treatment is individualized based on the location of pain, muscle tension, mobility limitations, related neck or shoulder symptoms, and patient goals.
For thoracic pain, medical acupuncture may help support pain modulation, muscle relaxation, circulation, nervous system regulation, reduced guarding in the upper and mid-back, improved comfort with breathing and movement, and stress-related tension patterns. Acupuncture is not a guaranteed cure, but can be part of a thoughtful conservative care plan.
Medical massage and myofascial release may be especially relevant for thoracic pain involving muscle tightness, fascial restriction, upper back trigger points, or posture-related stiffness.
Treatment may address pain between the shoulder blades, upper back muscle tension, rib cage and thoracic soft-tissue restriction, shoulder blade mobility, trigger point irritation, postural strain from sitting or driving, and compensation from neck, shoulder, or low back pain.
